 Permission Slips

We have created permission slips for Mr. and Mrs. Moon to sign. These permission slips gave us consent to film in their house. It acknowledged that their house would be used for a film as a school project, short film, and social media content. It also clarified that any damage would be the responsibility of MK Productions, there will be no illegal activities done on the property, and that the production team will treat the property with respect. Here is a copy of the forms that were read and completed by the owners of "The Moon House".
